As we wrap up 2026 and look ahead to 2027, health insurance is top of mind for many Americans. For some, the focus is on taking advantage of their 2026 benefits–especially if they’ve already hit their deductible. Others may be focusing on their 2027 benefit options, especially during the open enrollment period.
For Americans who struggle with addiction–or have loved ones facing addiction–understanding and utilizing their insurance coverage to help with rehab can be the difference between being able to get the help they need or not.
According to Forbes, about 28% of Americans have less than $1,000 in savings. And since the out-of-pocket cost for rehab can be in the tens of thousands of dollars without insurance coverage, it quickly becomes clear that quality rehab options will be out of reach for millions of people without the help of insurance.
In this article, we’ll take a closer look at how insurance coverage works for drug and alcohol rehab, as well as how Sana specifically helps you maximize your insurance coverage.
How Much Does Rehab Cost Without Insurance?
As you’d expect, there are many factors that can impact the cost of paying for rehab without insurance:
- What facility are you going to?
- Do you need a detox phase?
- Is it a residential facility, PHP (partial hospitalization program), or IOP (intensive outpatient program)?
- How long do you need care?
In addition to that, there may be a difference between cash pay (with no insurance at all) vs having insurance but choosing an out-of-network facility.
For a 30-day residential rehab stay, a 2026 secret shopper study showed an average cash pay price of $27,466.
While that may be attainable for some people, the reality is a price tag like that is simply not an option for most Americans.
How Much Does Rehab Cost with Insurance?
When it comes to paying for rehab with insurance, the numbers change significantly (thank goodness!). As we mentioned above, there are a wide range of factors that can impact what you should expect to pay. And when you add the complexities of insurance to the mix, there are even more variables:
- What facilities are in-network?
- Do you need to choose somewhere local/regional, or do you have the option to go out of the area?
- What is your deductible and out-of-pocket maximum, and how close are you to hitting those numbers at this point in the year?
The same study we referenced above showed an average cost of $5,500 for a 30-day residential stay at an in-network rehab facility.
Again, depending on your specific coverage, that may vary. At Sana, we’re often able to work with you and your insurance to find a way to get you or your loved one here at a cost you can afford.
Does Insurance Cover Alcohol Rehab?
Thankfully, many health insurance plans do cover rehab for alcohol. In fact, if you’ve purchased insurance through the government marketplace, every one of those plans is required to include coverage for substance abuse.
For plans outside of the government marketplace, here are some things that may impact the kind of coverage your insurance plan has for alcoholism:
- What state are you in? Some states have specific laws requiring coverage.
- Where is your insurance through? Large employers, small employers, and individual plans may all have different coverage.
- What kind of plan do you have? Plans that are “grandfathered” (in place since before the Affordable Care Act) and some large or self-funded employer plans aren’t required to include substance abuse coverage, though many do offer it.
- What level of care do you need? Even when a plan covers alcohol rehab, coverage can differ for detox, residential treatment, and outpatient programs. Some plans limit the number of days or visits.
- Is the facility in your plan’s network? In-network programs usually cost much less out of pocket. Out-of-network care may be partially covered or not covered at all.
- Does your plan require approval first? Many insurers require prior authorization or a doctor’s referral before they’ll pay for treatment.
Does Insurance Cover Drug Rehab?
This one we can keep short and sweet: Everything we said above regarding coverage for alcohol rehab also applies to rehab for a drug addiction. In most cases, insurers lump all this together under “substance abuse” coverage.
Insurance Plans that Cover Rehab
While it’s not possible to include an exhaustive list of all possible insurers or insurance plans that cover drug and alcohol rehab, there are several that we often work with here at Sana to make rehab possible for so many people.
At Sana, we are in network for all of these insurers:
- BlueCross BlueShield
- Cigna
- United Healthcare
- UBH
- UMR
- MVP
- First Health
- Tufts Health Plan
- Harvard Pilgrim Health Care
What “In Network” Means for You
When a treatment center is in network with your insurer, the two have a contract that sets the rates for care. For you, that usually means lower out-of-pocket costs, such as smaller copays and deductibles, and a more straightforward billing and approval process.
If Your Plan Isn’t Listed Above, Don’t Panic
It may simply mean Sana isn’t in network with your specific plan. That doesn’t mean you can’t use your insurance here. We also accept most out-of-network insurance and all private insurance, and our staff is skilled at helping you understand your benefits, work with your insurer, and find the most affordable way to get the care you need.
Insurance Verification for Rehab: Our Process
To find out more about your insurance coverage–whether you’re looking to maximize your 2026 benefits or trying to find the best plan for 2027–talking to the insurance company directly can be a good place to start:
- Call the number on your insurance card and ask specifically about alcohol or substance use disorder benefits.
- Ask about each level of care (detox, residential, partial hospitalization, intensive outpatient, and outpatient), plus your deductible, copays, and out-of-pocket maximum.
- Ask if prior authorization is required and which facilities are in network.
Or to make things simpler: Let us do that work for you.
Our admissions team can verify your insurance benefits and explain what your plan is likely to cover, including your estimated out-of-pocket costs, before you commit to anything. To make it quick, have your insurance card handy, including the member ID and the phone number on the back.
