Direct Care Specialist/LNA/LPN

SUMMARY/OBJECTIVE
Direct Care Specialists are a key part of Sana’s multidisciplinary team and play an extremely important role in making every patient’s stay as safe and comfortable as possible. The position is responsible for assisting patients with activities of daily living, therapeutic programming and socialization and creating a welcoming, therapeutic environment.

ESSENTIAL FUNCTIONS (include but are not limited to):

  • Assist in performing patient intakes, including interviewing patients to obtain basic history and performing baggage and property searches in accordance with established guidelines.
  • Address the daily non-therapeutic needs of patients using a calm, pleasant demeanor.
  • Orient new patients to the facility and services.
  • Assist with morning wake-up and evening bed checks.
  • Prepare to welcome new admissions by ensuring that all welcome materials are in place and rooms are clean and orderly.
  • Effectively communicate with patients using positive patient-centered language.
  • Accurately document patient information into the electronic medical record system.
  • Log laboratory specimens and prepare them for testing as indicated by the licensed staff.
  • Prepare treatment rooms for patient examinations, keeping the rooms neat and clean.
  • Clean and sterilize instruments and dispose of contaminated supplies.
  • Maintain inventory and order medical, lab and office supplies and equipment.
  • Report all observed patient concerns and changes in condition to a licensed staff member.
  • Assist with transportation of patients to ancillary departments or appointments as directed.
  • Facilitate ordering and pick up of prescriptions for clients.
  • Facilitate groups and activities for patients as directed.
  • Assist nursing staff with measuring vital signs, weight and height

KNOWLEDGE/SKILLS/ABILITIES (include but are not limited to):

QUALIFICATIONS

  • High School Diploma or equivalent required; Associate’s or Bachelor’s Degree preferred.
  • Active licensure in Vermont as a Licensed Nursing Assistant (LNA) preferred.
  • One or more years of experience working in a healthcare setting, preferably experience in a behavioral healthcare or substance use treatment setting

WORKING CONDITIONS AND PHYSICAL DEMANDS

Employee must be able to meet the following requirements with or without an accommodation:

  • Exposure to OSHA Category 1 tasks involving exposure to blood, body fluids, or tissues. All job-related tasks that involve an inherent potential for mucous membrane or skin contact with blood, body fluids or tissues, or a potential for spills or splashes of them are Category I tasks. Use of appropriate protective measures is required for every employee engaged in Category I tasks. Specific combinations of clothing and equipment are tailored for specific tasks. Registered Nurse

***We are currently hiring for all shifts, particularly nights and weekends, and offer generous pay differentials.***

SUMMARY
Nursing staff are responsible for daily patient care and working closely with the medical and clinical staff in the evaluation, treatment and care of patients. Duties include but are not limited to providing general nursing care, monitoring and safety, medication management, patient education and documentation.

ESSENTIAL FUNCTIONS (include but are not limited to):

  • Deliver direct patient care safely and effectively
  • Conduct, document and implement nursing assessments and care plans
  • Assist with patient admissions, discharges and transfers
  • Respond to patient care needs/issues and assists patients with activities of daily living
  • Collaborate with the multidisciplinary team in developing treatment plans, providing health information and maintaining patient medical records
  • Administer medications and carry out physician orders
  • Supervise and appropriately delegate patient care provided by medical assistants and non-licensed nursing personnel
  • Record and report all observed symptoms, reactions to treatments and changes in patient progress and needs
  • Provide in-service education to staff
  • Facilitate psychoeducation groups

KNOWLEDGE/SKILLS/ABILITIES (include but are not limited to):

  • Knowledge of and ability to apply professional nursing theory, practice and ethics.
  • Ability to maintain strict patient confidentiality and adhere to federal standards under the Code of Federal Regulations 42 Part 2 and HIPAA.
  • Computer literacy/basic computer skills and ability to use an Electronic Health Record system.
  • Ability to work independently under medical protocol.
  • Knowledge of evidenced-based substance use disorders and behavioral medicine treatments and methodologies preferred.

QUALIFICATIONS

  • Registered nurse (RN) with active State of Vermont licensure.
  • Bachelor of Science in Nursing preferred.

Counselor

SUMMARY
The Counselor is responsible for screening, assessing and providing a range of treatment services to patients. Under the supervision of the VP of Clinical Affairs and as part of a multidisciplinary team, the Counselor implements evidence-based treatments, including individual, couples, family and group counseling, case management, patient advocacy, psychoeducation and crisis intervention as needed. As the organization grows, the hope is to have this position grow into a full-time role.

ESSENTIAL FUNCTIONS (include but are not limited to):

  • Conduct clinical screenings and assessments through observation, testing, interviewing and collateral information
  • Develop, review and update individualized patient treatment plans in collaboration with multidisciplinary treatment team
  • Coordinate all treatment services and provide evidenced-based clinical interventions
  • Provide individual, couples, group and family counseling services using evidence-based treatment modalities
  • Document all clinical interventions and communicate patient needs to treatment team in a timely manner
  • Communicate with referral sources, outside providers, peer support networks and patient loved ones to coordinate care
  • Collaborate with the Continued Care Specialist and other treatment team members to develop a comprehensive aftercare plan for each patient

QUALIFICATIONS:

  • Master’s degree in Social Work, Psychology or another licensure-eligible related field
  • Active State of Vermont licensure as one or more of the following: LADC, LICSW, LMFT, LCMHC and/or Psychologist (Master’s Level)
  • Two or more years of experience as a therapist/clinician in the substance use disorder or behavioral health field

WORKING CONDITIONS AND PHYSICAL DEMANDS:

  • Prolonged periods sitting at a desk and working on a computer
  • Perform weight-related functions, including lifting up to 15 lbs.

Psychologist

SUMMARY
The Psychologist is responsible for providing a full range of counseling and assessment services to patients. Under the direction of the Medical Director and as part of a multidisciplinary team, the Psychologist administers and provides intake assessments, treatment planning, individual psychotherapy, psychological testing and assessment, group psychotherapy, trainee supervision, case conference consultation and crisis intervention as needed.

ESSENTIAL FUNCTIONS (include but are not limited to):

  • Assess patient psychological needs, abilities and behaviors by using appropriate evidence-based psychological screenings and tests.
  • Administer, score, document and interpret a variety of assessment tools.
  • Collaborate with multidisciplinary team members to develop, maintain, implement and update treatment plans using evidence-based psychological interventions.
  • Facilitate group and individual psychotherapy.
  • Provide timely documentation for assessment and treatment activities.
  • Provide crisis assessment and counseling as needed

QUALIFICATIONS:

  • Doctorate in Psychology from a graduate program accredited by the American Psychological Association
  • Active State of Vermont license as a Psychologist
  • Three or more years of experience in substance use disorder and behavioral health field
